Repeat Prescriptions

If you have not yet registered for our online Patient Access service, please try it out as it is usually the most efficient way to request your regular medications.  If you prefer not to use this service use this form to make your request.  

You can ask your local pharmacist to request your repeat prescriptions for you, if you prefer. 

For clinical safety reasons we do not accept requests for prescriptions by telephone. 

Please only order the items you need to prevent waste.

Use this form to request your repeat prescriptions

Your prescription will be available for collection after 3pm the following working day, excluding weekends and Bank Holiday (providing there are no queries or problems with medication) either at the surgery, or from the pharmacy you have nominated. We will post the prescription to you if you provide a stamped self-addressed envelope.

If you have a question about your medication, side effects or how to take it, please consult your local pharmacist, who has great expertise in this area.

Use our online form and we will get back to you within 2 working days.

Please ensure that the review date on your current prescription has not expired. If it has, please make a telephone consultation with your usual GP or practice pharmacist. If you are unsure as to whether a review is needed, please make a telephone consultation.

If you have passed the medication review date without having your review you risk not being able to get your medications until the review is done. Please help the doctor to keep a check on your condition by contacting the surgery when requested.

The GP may need to contact you about your repeat prescription, please ensure that we have an up to date telephone number where we can contact you when you request a prescription.
