It’s important to tell us if you change your personal details like name, address or contact information
Changing your contact details
To update details like your name, address, phone numbers or email address:
- fill out a change of contact details form
- phone or visit the surgery during our opening hours
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
We can now keep patients who are already registered with us, even if they move to a new home outside our catchment area, as long as they still live in Wandsworth. We will look at each person's situation and decide if they can stay registered with us.
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your date of birth or name
You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
